On February 28, 2017, Great Lakes Health Connect and HealthCare Synergy are partnering with St. Joseph Mercy Oakland Hospital in Pontiac to bring home health agencies an unprecedented opportunity to learn and understand how important interoperability and the secure exchange of health data among care providers will become reality in 2017.
Many resources that hospitals and physicians have become accustom to, and rely on, have never been introduced or explained to home health agencies. These resources range from understanding and accessing a patient’s longitudinal care record, thus understanding more information about a patient before an initial assessment, to secure exchange of referral information.
Home health is a pivotal care option for the expanding Medicare population in the state, and yet there has always been a lack of effort to bring independent providers the same technology and patient information that exists in the acute care arena. The afternoon’s workshop will show providers real world options that are in place today and federal regulations that impact the use of this technology, especially under the MACRA act for Physicians, Hospitals, and ACO’s.
This event is open to any and all healthcare providers at no charge, but is focused for those providing Medicare covered Home Health Care.
