Hospitals, medical offices, and many other parts of the care continuum are now required and offered incentives to use electronic health records to organize and share information between organizations more efficiently.
Community and social service organizations have been historically exempt from this requirement. But as the healthcare industry moves from volume to value based care there is a need to coordinate services beyond bedside care and doctors office visits.
Coordinating medical care with social determinants of health services demands that processes, communication tools, and systems be examined. Please join the GLHC Community Advisory Focus Group at our next meeting to discuss how the entire care continuum can work together.
Great Lakes Health Connect has the experience to convene all stakeholders and to create a solution, meeting the needs of the entire community.
